Name of Forum: (obviously, we need a name)
Personal Reason: (this is what YOU think)
World Reason: (this is what you think the WORLD/PEOPLE thinks)
Activity: (how are you going to be active in this forum when it's created)
Time: (how much time do you want the forum to last [ex: a month, forever, etc])
Moderator: (do you want to be a moderator of this forum)
-Reason: (state reason[s] for you being a moderator)
-Activity: (state how long you'll stay in this forum when you're a mod)
-Manage: (how are you going to manage the forum)
